Notice of Privacy Practices

This notice applies to all professionals, staff, affiliates, and business associates of Plastic Surgery Specialists of South Florida, who handle your protected health information (PHI) in the course of treatment, payment, and health care operations.

We understand that medical information about you is personal and sensitive. We are committed to safeguarding your PHI while providing high-quality medical care. This notice explains how we use and disclose PHI, your rights under the law, and how you can access your information.
3. How We May Use and Disclose PHI
a. For Treatment, Payment & Health Care Operations (TPO)

Treatment: To diagnose, manage, coordinate care with other providers, or refer you to specialists.

Payment: We may share PHI with your health insurer or third-party payers to secure payment.

Operations: Includes auditing, credentialing, quality review, staff training, and marketing that informs you of treatment alternatives or services.
b. Other Permitted Disclosures (without written authorization)

Family involvement and notification in your care (unless you object).

Required by law (e.g. public health reporting, legal proceedings, law enforcement, organ donation, workers’ compensation).

Research, if Institutional Review Board (IRB)-approved and privacy safeguards are in place.
c. Uses Requiring Your Written Authorization
Marketing communications, sale of PHI, psychotherapy notes, or other uses not specified under permitted disclosures. You may revoke this authorization in writing at any time.
4. Your Rights Regarding PHI
You have rights regarding your protected health information, including:
